2. Applicant Representations I represent that all information provided in this application is true, complete, and accurate. I further represent that I am authorized to act on behalf of the entity listed in the application and to bind such entity in commercial discussions and agreements.

3. Binding Intent & Confidentiality I understand that by submitting this application, I am seeking to establish a commercial relationship with PureAxis Health. The representations I have made are accurate to the best of my knowledge. I agree not to disclose any nonpublic information obtained through this application process to any third parties without the express written consent of PureAxis Health. I further represent and warrant that I am applying in good faith, solely for the purpose of becoming a PureAxis Health partner, and not for any competitive or unauthorized purpose.
